Outside of employer-sponsored insurance, you have two choices: ACA plans or Private Health Insurance. ACA plans are ideal for individuals with low income or major pre-existing conditions. Private plans are ideal for healthy individuals who want low to no out-of-pocket costs on a PPO network.
HMO (Health Maintenance Organization): A plan with limited provider networks that requires referrals from your chosen primary care physician (PCP) for any specialist services.
EPO (Exclusive Provider Organization): Like HMOs, EPOs cover only in-network care. However, they usually have a larger network of providers and may or may not require referrals to access specialists.
PPO (Preferred Provider Organization): Offers nationwide coverage with the freedom to select your doctors. PPO networks permit you to see specialists and out-of-network providers without needing a referral.
